Student Health Service – Forms Available

Dear Parents,

The Student Health Service of the Department of Health is now enrolling students for the 2019-2020 school year.

Forms are now available for pick up in the main office in the drawers under the trophies.

Please return all completed forms back to the main office by Tuesday 10 September 2019. After this date, you will need to fax completed forms directly to the Department of Health at 2915 1499.

Year 8 students are also eligible for a Hearing Screen Test. Details for this test are also available in the main office in the drawers under the trophies.
